Role Description: At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. They will be an effective cross team leader, working side by side with Treehouse members and collaborating with providers, care teams, operations team members. Content areas for the Director will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical social workers, and others. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, regions, and centers.

Pay Range: The typical pay range for this role is: $100,000.00 - $231,540.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company's equity award program.
